Please Select Parend ID Proof File with Signed Copy – Birth Name Inclusion Error
I’m trying to submit an application for Birth Name Inclusion on the Chennai Corporation website. I have uploaded the required documents in all the respective fields, including the signed parent ID proof.
However, when I click Submit, I get the following error:
“Please Select Parent ID Proof File with Signed Copy”
The documents I have uploaded are:
- Scanned copy of the parents’ photo ID proof with signature
- Scanned copy of the declaration from the parents with signature
- Date of birth proof certificate
The parent ID proof has already been uploaded correctly, but the website still shows the same error.
Has anyone faced this issue? What should I do to get the application submitted successfully?

Try uploading a document in all four fields, even if the fourth field is marked as “Any Other Proof.”
For the “Any Other Proof” field, if you don’t have another supporting document, you can upload the same parents’ ID proof with signature or the signed declaration again.
Then try submitting the application once more. This may help if the portal is treating all four document fields as mandatory.
